Policy change for vaccination booklet requests effective November 1, 2025

ORANJESTAD—The Youth Health Care Department of the Public Health Service announces a change in the policy for requesting vaccination booklets effective November 1, 2025.

Do you need a duplicate vaccination booklet?

The new procedure is below.

Procedure for requesting a duplicate vaccination booklet:

  1. The cost for issuing a new vaccination booklet is Afl. 10.00. You must transfer this amount to the account number of the Public Health Department at Aruba Bank: #424499.

    When transferring, please include the following in the description:

    a.    Vaccination booklet;
    b.    Child's full name;
    c.    Child's date of birth;
    d.    Child's parents' name.
    e.    If you are unable to make a bank transfer, you can pay in cash at DVG-Financiën, located at Dr. Oduberstraat 2 (DVG) in Oranjestad.

  2. Send proof of payment to jgz@dvg.aw with the subject line: Proof of payment for vaccination booklet.
  3. Then schedule an appointment to collect the booklet by calling 522-4200.
  4. You can collect the vaccination booklet at Cumana 80-D, from 8:00 AM to 11:00 AM and from 1:30 PM to 3:30 PM (Monday through Friday).
